We want to thank you for choosing Hayyan Healthcare as your health care provider. In order to give you and all of our patients, the best possible care, we request that you review our policy regarding missed appointments. A missed appointment is when you fail to show up for an allotted appointment time, without a phone call or cancellation notice of at least 24-hours. Please remember that we have reserved appointment times especially for you. Therefore, we request at least a 24 hour notice in order to reschedule your appointment. This will enable us to offer your cancelled time to other patients. If you are unable to keep your scheduled appointment time, please call our office at least 24-hours in advance in order to avoid a missed appointment fee. Your phone call is critical in helping us provide continuous care to all of our valued patients.

If you fail to give us 24 hours notice of your missed appointment the following charges will apply:

All Physical Examinations

Appointments other than Physicals:

1st missed appointment
You will be notified that you have missed your appointment and advised that on the second missed appointment you will be charged

2nd missed appointment
$50.00 charge will apply

If you arrive more than 5 minutes late for your appointment you will be deemed to have missed your appointment. If the physician’s schedule allows we will accommodate you in the next available appointment time for that day. Priority will be given to patients arriving on time and you may have to be worked in
between them. This may mean you will have a considerable wait.

If this is not convenient, you may choose to reschedule and the above missed appointment fees will apply.

Test Results
Due to the volume of test results coming to the clinic each day, normal results will not be reported to the patient. We will contact patients with abnormal results or for tests that require a follow-up visit. Pregnancy test results will be called to the patient. If you wish to discuss test results with your physician, please book a follow-up appointment for one to two weeks after the tests are done.

Telephone Calls
If you have a medical problem or question you wish to discuss, please make an appointment or come to the walk-in. Most medical questions need to be dealt with by a physician in order to assess the problem and document matters properly.
